Important Information
New Patients: Please complete your medical history, insurance information, and intake forms in advance.
Insurance: We accept a wide range of insurance plans, including most major commercial insurers, local hospital system insurance, Medicare, and Tricare. We do not currently accept Medicaid plans. Click here for a more detailed list of plans we accept.
Cancellation Policy: We kindly ask for at least 24 hours' notice if you need to cancel your appointment. A $25 fee may apply for missed medical appointments, and a $75 fee for missed surgical or cosmetic appointments. Exceptions may be made in cases of extenuating circumstances.
Credit Card on File Policy: We require a credit card to be kept on file. Your card may be charged after a specified period after your statement is sent if payment has not been made. We will make an effort to notify you before any charges are applied. Please refer to our consent form or FAQ for full details.
